I understand that the National Youth Commission into Youth Homelessness [NYC] is an independent community inquiry into youth homelessness in Australia. I have been sufficiently informed about the NYC terms of reference and operational guidelines. My participation and contribution of information to the National Youth Commission is voluntary and on the following basis:

(a) agree that any information provided at public hearings or publicly online may be used, with the appropriate reference, for the purpose of the NYC report or in NYC publications arising from the inquiry.
(b) agree that information provided in confidence or privately and informally out of session may also be used, but in a way that does not identify authorship of the comments or information.
(c) understand that the NYC will securely store all submissions, transcripts and de-identified confidential or private information/ comments for a period of no less than five years after which the NYC archive may be deposited in a university or state or national library archive.
